Important information for our patients and the services we provide at Greenvale Family Medical Practice.

Calling your GP
You may contact your doctor by telephone during normal surgery hours.
A message will be taken and passed on to your doctor. Your doctor will
contact you as soon as they are able. In case of an emergency the call
will be put through straight away.

Test results
For information security and privacy, your test results are not given out
over the telephone. We request that all patients book an appointment to return to the centre to discuss their results with their doctor.

Recalls and reminders
The Doctors in this practice routinely send recalls and reminders to
patients by SMS for follow up consultation and the review of test results.
If you do not wish to receive these reminders, please advise your Doctor
or reception team.

Electronic communication
Our practice values your privacy. At times we may email you from our secure email system or practice software. You may also receive SMS messages if you have opted-in to receive them.

Transfer of patient medical records
If you require your medical records to be transferred to our practice, a signed consent form will be necessary to allow our practice to obtain your medical file. Please see receptionist to complete the consent form.

Fees and appointments
Our practice is a bulk billing practice for patients who hold a current medicare card. If you do not hold a current medicare card a fee will apply. Our standard appointments are 10 minutes but if you feel you may need longer or have multiple issues you need addressed, feel free to request a longer consultation upon booking your appointment

Home visits
Doctors at our practice are available for home visits for regular patients of the practice. If you require a home visit please call our reception staff who will speak with your Doctor to arrange same.

After hours service
When our practice is closed your ongoing medical care is important to us. If the matter is urgent please call 000. If you require medical attention please call the clinic and your call will be triaged and a decision will be made by your Doctor regarding after hours care.

Translator service
All Doctors at our practice are registered with T.I.S (National Interpreter Service) if you require assistance during your consultation time with our Doctors please let reception staff know and an interpreter can be organised.

Privacy policy
Your privacy and confidentiality is paramount at our practice. Your medical records are confidential. It is the policy of this practice to maintain security of personal information at all times and to ensure that this information
is only available to authorized members of staff. A copy of our Privacy Policy is displayed in our waiting room or available from the practice team.
